Enrollment Paperwork: All families are required to fill out, in entirety the following documents prior to the first day of enrollment. Families will be asked to update documents every six months, or whenever a change in address/phone number, or any significant factor occurs.
Families are invited, and encouraged to tour the center prior to enrollment. In order to be familiar with all our policies and procedures, families are asked to review and agree to the policies and procedures outline in the parent handbook.

Child Health Report - Completed by a physician and updated appropriately. (Every 6 months for a child age 2 and under, every year for a child age 3 and older. A copy of updated shot records should be provided after each well-visit.
