Enrollment Paperwork: All families are required to fill out, in entirety the following documents prior to the first day of enrollment. Families will be asked to update documents every six months, or whenever a change in address/phone number, or any significant factor occurs. You may download and print the forms, here, or stop by for an enrollment packet.
Families are invited, and highly encouraged to tour the center prior to enrollment. A tour will ensure that you receive all necessary information prior to your first day. In order to be familiar with all our policies and procedures, families are asked to review and agree to the policies and procedures outline in the parent handbook.

Enrollment Document - Completed in entirety, all sections must be completed in order to be accepted.

Handbook Agreement Form - An electronic copy of the parent handbook will be provided to you.

Child Health Report - Completed by a physician and updated appropriately. (Every 6 months for a child age 2 and under, every year for a child age 3 and older. A copy of updated shot records should be provided after each well-visit.

Emergency Information - Completed by the enrolling parent/guardian and updated every six months, or any time a change occurs. Document must be completed in entirety including addresses, phone numbers and relationships.

Social/Emotional Development Form: Completing this form will allow our teachers to get to know your child and family.
